Long Term Conditions

Long  term conditions are reviewed in the month of your date of birth. This helps us and you plan when you will need to be seen. Our slogan is: “Happy Birthday to you, your review is now due”.

Asthma Clinics

Regular check-ups and advice from practice nurses on lifestyle, inhaler techniques and medication. If you have been advised by the surgery to submit an annual review of your asthma symptoms, please use our Asthma Review form.

Diabetes Clinics

Patients will be invited for a check up, either for an annual diabetic review or a medication and blood review.

COPD Clinics

Regular check-ups and advice from practice nurses on lifestyle, inhaler techniques and medication. If you have been advised by the surgery to submit an annual review of your asthma symptoms, please use our COPD Review form

Child Vaccination Clinics

These clinics are by appointment and are held by the practice nurses. (Please note that 8 week checks are held by the doctors and the practice nurses, and may be at varying times.)

Contraception, Coil and Implant Fitting Clinics

If you have been advised by the surgery to submit a contraceptive pill review, please use the relevant form below:

Coils and implants are fitted by Dr Acharya. You must make a routine appointment to see Dr. Acharya  who will discuss if this form of contraception is appropriate, and will then advise you to book an appointment at reception to proceed.

Minor Surgery Clinics

We provide a full range of minor surgical procedures including removal of moles, warts and skin lesions. We also offer joint infections. Please make an appointment with either Dr Low, Dr Taylor or Dr Olmos Berrio to discuss your concerns.

Minor Injuries

To prevent unnecessary hospital attendances and to provide care closer to home, we can offer treatment for some minor injuries. This includes treatment to superficial lacerations including gluing, removal of minor foreign bodies from the skin, ear or nose, treatment burns and bites.

We would advise all patients with minor injuries to call the surgery for advice before attending in order to sign post you to the correct service.

Head injuries, involving loss of consciousness, should go immediately to A & E.

Smear Clinics

Available for various appointment times. Please make an appointment with a nurse to have your smear test.

NHS Health Check Clinics

Patients aged 40-74, without pre-existing diabetes, hypertension or kidney disease, are entitled to a free NHS Health check every 5 years. Appointments are available with one of our team.

Counselling Services

Counselling is accessed and arranged via one of our clinicians. Alternatively, you can self refer for counselling services.

Phlebotomy Clinics

Due to the continued pressures on primary care, it is with regret that, as a surgery, we are not able to offer phlebotomy services to some of our patients. If you are able, you will be asked to go to the hospital for your bloods. We will ask which hospital you prefer and a request form will be raised for you to collect. Appointments for blood tests need to be made by accessing the relevant hospital’s website.

Request forms should be taken with you to your appointment.

Blood tests requested by the hospital should be carried out at the requesting hospital.

Travel Vaccinations

Please complete our Travel Risk Assessment form at least 6 weeks before you travel. The form will be reviewed by the practice nurse who will check your medical records to identify which of the requested vaccines you need. Please call the practice one week after submitting your application, to find out what vaccines are required and to book a suitable appointment.

It is important to complete the form as early as possible, as an appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least 3 weeks before you travel to allow the vaccines to become effective.

Please note that the practice does noes supply vaccines that are currently not funded by the NHS.
